To ensure the safety of everyone using the lab, all users need to complete the safety form below. Make sure to complete required online EH&S trainings before starting the form.

Trouble logging in?
If you come across a message that says you do not have permission to access the form, follow these steps:
                   1.  Sign out of your gmail account (Usually on the upper right corner of browser)
                   2.  Sign in with your @stanford.edu account.This google form contains upload options, and is therefore only accessible to the Stanford organization.
